I removed the training wire from this tree about two months ago, so the shape is slightly off-balance and needs readjustment; I also just gave it a trim today. It is a mature tree, but I recently decided to reduce its size, resulting in a cut at the "elbow" section—right where you can see new shoots emerging. The reason I left that stub there is to help the wound heal faster, though you are free to cut it off if you find it unsightly. The tree is flawless from base to tip, and looking at it, you can easily visualize how future maintenance will simply require occasional trimming.
